Between surviving the disease and returning to normal life, there lies a path of thousands of small victories. Physical therapy helps people achieve these victories, one step at a time.
There is a moment after a heart attack or stroke that few people talk about. This is not the moment the ambulance arrives. This is not the moment when doctors say that the patient is out of danger. This is the moment when the hospital doors close behind you, you return home, and everyone expects life to gradually return to normal.
But what does “normal” mean?
For a person who has had a stroke, this may mean learning to walk again, using a hand that suddenly feels unfamiliar, finding balance, or simply getting out of bed without someone holding their hand. For someone who has had a heart attack, this may mean walking outside for the first time without constantly wondering if their heart can handle it.
And sometimes the hardest thing is not the physical limitations. This is fear. – What if it happens again? This question may remain with a person long after they have left the hospital. This is where physical therapy becomes more than just a set of exercises.
What does rehabilitation include?
We often think of rehabilitation as someone working out hard in the gym, lifting weights, or walking on a treadmill. The reality may be much calmer.
For a stroke survivor, the first goal may be to simply sit up straight without support. Then stand. Then take a few steps. For family viewing, this may seem like very little. For the person doing this, it can be like climbing a mountain.
At such times, a physiotherapist is at your service. Not just to count reps or measure strength, but to help someone understand that progress doesn’t always have to be dramatic. Sometimes progress is just one extra step. Sometimes it stands ten seconds longer. Sometimes he can walk to the toilet without asking for help. And sometimes you just need to find the confidence to try.
What patients go through
Imagine being a person who went to work, played with your children, exercised regularly, or climbed four flights of stairs without thinking about it. Then you will have a heart attack. Suddenly your heartbeat becomes something you notice constantly. You begin to question every sensation. This is fine? Am I too tired? Is my heart rate too high? Should I stop?
This fear can cause people to become overly cautious and inactive. Cardiac rehabilitation can help break this vicious cycle. With appropriate medical assessment and monitoring, patients can gradually return to physical activity, improve their physical performance, and learn to manage their cardiovascular health with greater confidence. The role of the physical therapist is not to simply say, “Push harder.” It means, “Let’s understand what your body can safely do today and start from there.”
Not just for the elderly
A stroke or heart attack is often something that happens later in life, after age 60 or so. But when it happens to a young person, the shock can be enormous. Someone in their 30s or 40s may suddenly begin to worry about things they never expected to question. Will I be able to return to work? Can I take care of my children? Can I travel? Can I play sports again? Will I be able to return to my favorite sport?
Outwardly, they may look completely healthy. But inside, they may be dealing with fatigue, weakness, uncertainty, and fear of what will happen next. This is why rehabilitation cannot be based simply on a person’s age or diagnosis. A younger patient may have to return to physically demanding work. The athlete may want to return to competition. A parent may just need enough energy to play with their children. Rehabilitation must correspond to the life to which the person wants to return.
The patient is not a diagnosis. This may be one of the most important things a physical therapist will learn. The medical report may say “stroke.” It could be a heart attack. It may include numbers, test results, and medications. But none of these words explain who the patient is. They don’t tell you that the patient enjoys morning walks. They don’t tell you that someone has a baby at home. They don’t tell you that being able to climb the stairs on their own means more to that person than anything else. Good rehabilitation starts with a simple question: “What do you want to get back to?” The answer may be different for each person. And this response becomes an important part of treatment.
Prevention goals
Physiotherapy also aims to prevent the next episode. Rehabilitation should begin not only after the crisis. Physical therapists also play an important role in helping people become more physically active, develop healthier movement habits and reduce preventable health risks. Physical activity is an important part of cardiovascular health, but exercise is not a recipe that can simply be copied from one person to another.
The right type, intensity, and amount of activity varies from person to person. For a person recovering from a heart attack, exercise may require appropriate medical guidance and monitoring of symptoms, heart rate, blood pressure, and exercise capacity. For a person recovering from a stroke, rehabilitation may include improving balance, strength, coordination, walking, and daily functional activities.
The goal is not to get everyone to do more. The goal is to help each person do what works for them—safely and consistently. The best thing a physical therapist can do to restore you is not movement. It’s that part of someone’s life that they thought was gone forever.
We often say that physical therapists help people move. But perhaps that doesn’t quite tell the whole story. Physical therapists help people get back to the things that make life feel like living.
